Blizzard has mentioned that Diablo 3 will be the end of the current arc of the series.
Cain is normally depicted as the sort of 'narrator' of the game. But in the Diablo 3 trailer, the narrator was a new character named Leah.
Leah appears to be a NPC directly related with Cain and is heavily speculated to be an important character.
Blizzard doesn't shy away from killing off major characters to make way for new ones.
Basically, the way I see it, Leah may be being groomed to replace Deckard Cain as the game's narrator in subsequent titles. The reason for this is time. With twenty years having passed since the first two games, Cain is without doubt remarkably old given the average life span in-game. Given that Cain probably won't live long enough for the next game should significant time pass between them (such as in the case between Diablo 2 and 3), it could be seen as better use of the character to have him killed off memorably to further the plot and add 'drama'.

3. speculation? I haven't heard this, even so it doesn't suggest his death.
4. I don't think it has anything to do with 'making way' and more to show the brutality of sanctuary.
I actually do think Cain will die, perhaps the PC will even see it as being partially their fault. Somehow I think this Leah will believe it to be hers, prolly 'cause she was off with the PC.
One of the questions/complaints in the StarCraft II lore panal was about killing npcs and bringing them back all the time in Blizzard's games. (wow:kel'thud, syilvannos? - starcraft: phoenix, kerrigan)
Even if Cain dies in D3 you can expect him to come back at some point, and with wings on his back probably!
Look at the last page of Cain's diary, it's slashed with claws. I think Cain turned into a prime evil. I always knew he was evil (switching people's Stones of Jordan for Manald Heals during the "identification process", etc...)

All of my stones of jordan have been identified by scrolls...
But..... why would Cain need the 2,000,000,000 Sojs that everyone has found? He's not that selfish.... For the dclone perhaps? Not likely..... unless he pokes its' eyes out with his walking cain and trips him a few times into the river of flame.
As for Cain being evil.... I don't know.... it's plausable.... but it gives me the same feeling as Tyrael being evil... and that as sour as Cain's toes..... those dirty.... dirty.... sandles......
Cain carries something that LOOKS like a shard of the worldstone which is actually his future soulstone He will trap himself into that and possess Diablo himself, that's why it won't be "Diablo IV" but "Cain I"
