A titan puts all of his points into strength, unless he needs dex for his weapons. No vita. Vita would make him a "balanced barb" Yuck. We want damage, not n00bish stuff like "vitality" YUCK!
We want high ww dmg, enough to slaughter. We wont want to get slaughtered though.
Anyhow- the gear.

20 shout
some to nat res, iron skin, etc.
The stats-
88 dex after + stats
rest to strength
no vita you nub! (ok, if you want to you can add a little here, but i dont reccomend it. Its your barb tho)
The gear-
High damage lance with 75 durablity. I would rather have a 300 lance w/ 75 dura than a 350 w/ 25 dura
Gskin- resists, defense. a godly orn beats this
Angelic ammy and 1 ring- ar, so you hit more often. 2 angelic rings are overkill
Sigons helm + gloves- life steal, ar. I said 2 angelic rings are overkill, true. 1 angelic + sigons is perfect.
Rare war belt w/ high def, res, life, strength
Gob toes- cb
And thats it.
Couple things-
I usually prefer a mart on my pvm barbs. On a titan i prefer lance. Why? with less life the added range is very helpful, you kill them before they get close to you. Its also cooler owning with a lance as opposed to a mart.
With huge damage (were talking 3k+ ww) and decent resists, bo, and life steal... you wont die. Dont worry about base vita.. if ur on sc
since you won't focus much on resistance, why not blinkbat's form for armor? increase your run/walk speed, and 40% to faster hit recovery! isn't that key for two-handed users?

Blinkbats is the worst possible armour for a pvm ww barb i can think of
r/w is bad, fhr is not needed, awful defense
Resists ARE needed, at least have them posotive
Oh, and i forgot to post what other ring to use- if you need mana steal, have mana steal. If you can pskull your lance, then use something else
My friend did an all str lancer.... I would prefer to have some vit for this though.
I prefer axes for high str BvB, but for high str PvM, I prefer martels. If you do go all str, try to get alot of life off items. You can get enough to survive with good items, but you wouldn't be able to do river runs all vit/low res. I would go for flayer runs up to 83 or 84, then probably stop. Trav runs would work if I had some more res gear.
Assuming:
50 all from natural resist
35 from gskin
20 from ammy
20 from rings/boots
Use sigons gloves/helm for AR and leech
you could probably own trav with this. My ladder maul mfer is 85 now and has about 260 base str, rest vit, which is nice. No gskin though, so maxing resists is a pain in the ***.
