Actually quite simple...

You save Cain from the Cathedral..and get started on collecting the fragments...as part of getting the crown an extra task is added. Cain, Leah, and yourself go from Old tristram to new tristram on the road..looking for some lost text fragments. As yu travel you go by a special rift...

Suddenly a horn comes out from the rift...impaling Cain...behind it steps the biggest, shiniest, prettiest pony you've ever seen. It whinnies in delight, rainbows shooting out from the horn...Cain dies on the horn..as the pony disappears back into the portal. A cow stands up and says he can get you into the plane of the pony...but you must collect the ingredients and a special recipe..and so starts your real reason for playing the game...to collect all the ingredients to get into whimsyshire...killing the demons is just a side mission you happen to do as well.
