The biggest problem that I can see, beside setting fire to everything, is putting a piece of molten marshmallow in your mouth.
It's better to melt chocolate and dip the marshmallow in that! Chocolate fondue anyone?

Zippo-type lighters burn Naphtha, a semi-heavy, industrially produced petrochemical product. Bic lighters burn Butane, C4H10. Campfires burn wood. Randomly obtained from the wilderness.
Which do you think is more likely to contain kill-you sorts of compounds?

Butane and naphtha consist mainly of carbon and hydrogen, and they burn into CO2 and H2O which is perfectly safe as long as you don't inhale large amounts of smoke. Sniffing the unburned gas is pretty unhealthy though. It's perfectly safe to eat marshmellows cooked on a lighter. If you keep the lighter lit for a very long time there might be a small risk it will explode, so be careful.
A lighter usually doesn't produce a flame which burns all carbon. Only the hydrogenium might be burned fully, else the flame would look like one of a welder or a blowtorch. I guess a lighter produces a couple of nasty substances, in particular those which don't burn butane, like Zippos. They use gasoline AFAIK.

Campfires probably produce similar substances (including benzene compounds which are poisonous and can cause cancer), but I guess it's less unhealthy than a lighter. It's probably better to use charcoal.

Marsh mellows don't have to get that hot, you could get away with using a toaster oven and a skewer, if you could trick it into staying hot while the door is open.
