Only dogs return LoH now, and the poison ones have double the proc coefficient of the burning ones.
But it's still so low, I'm considering just taking the Gargantuan for pure face-tankiness, maybe with the stun rune.
RoT spammed with an acid burn seems good, but I'm wodnering if I could stack my IAS high enough to only spam toads, take advantage of that mana, and have a Spider Queen (which now triggers LoH!!) in place of acid cloud.

I am thinking that my 1.04 tank doctor build will be using dogs/gargantuan as the primary tank mechanism and LOH as a "Backup".

So I have been building the tank build and so far have been going through just fine in Inferno without many dogs dying if ever. However being a tank you rarely kill things, and since I play solo a lot, I got a lot of tank gear and crap loads of Thorns damage (about 16k thorns). They tank everything and at the same time things die. (i have about 75k life)
Also, I've been using the Leeching Beasts rune... So far it appears that since they do all the thorns damage, they are healing for 50% of that damage. Is that true? Because I figured Inferno would pose a much greater challenge... I'm feelin' lazy with this build.
I don;t know about leeching/thorns, but Fierce Loyalty gives them your rege. I have about 700 lfie regen and they seems to survive really well with my high armor/res.
Also, Acid Cloud - Slow Burn is much better than Spider Queen. Just tried it in practice. Forget what I said, use Acid Cloud, don't use Manitou either. Both aren't too great in practice.
I agree with this but I would extend it to say that any acid cloud rune is better. Spider Queen/Manitou are nice passive/free damage adders if you have a skill slot you don't need for something else but they're not worth if you don't have a real mana-burning nuke of some kind.
