Cause of demise: Connection lost to Battle.net/Disconnected, or whatever. I just got into a fight against a champion pack in the Desolate Sands and boom, connection lost. I relogged into the game, and my other character was waiting for me in the middle.
I'm not giving up to my journey to HC lvl60, but maybe I'll wait for an other try until the servers become more stable.
Also, I can't find the option to make text red.

Cause of demise: While engaging the champions pack my PC froze. After restart i found out my Barb was dead. A bit sad since Act II was going so darn easy - there was nothing that could kill me there. Well - my GPU drivers did that for the monsters :(

Cause of demise: Was mfing in random public games until by coincidence a really awesome group got together and we started pushing towards the end of Act3. Had some awesome tanks and dps - progress was slow but very safe and steady. Slew endless demons, champions, and bosses and were almost up to Azmodan in the core of arreat.
One last champion pack got in the way before Azmoda'n's personal lair. Those spider-dog like things. Desecrator, Frozen, Vortex. For some random reason I had a nasty hunch I will get vortexed into an ice orb which will hurt... Thankfully, that didn't happen.
Instead, I got vortexed into the middle of the three champions, right into an ice orb on top of desecration, and then chain frozen 2 more times in a row. Went from full buffs and full health to zero over a course of five-ten seconds while being permanently frozen without a spare millisecond chance to do anything. Couldn't teleport, couldn't knock them away, couldn't activate diamond on skin. Nothing at all as I watched health drop, spamming every button, hoping the damn freeze would go away a slight moment before I die. Turns out, it didn't work out.
No idea what I could have done against something like that other have a ton more gear - probably wouldn't have kept me alive anyway as I was by no means under-geared for the content. Essentially 1st toon. 160 hours. Solo'd up to half way through Hell act3. 12k dps. 60% armor. 45% rez all. 43k health.
Not sure how I feel now lol - Kinda relieved, oddly enough o_O.
Debating with myself whether I would have rather died to lag or this. I'm thinking lag - that I can blame on crap happens. This just feels like cheap game design. Owell.
I'll remake later on; was a good run. Needed a break from constantly playing since launch anyway – hardcore is a good addiction breaker. Time to go sleep early for once ^_^

Cause of demise: My life went down during a fight while I was surrounded by monsters ; I hit several times the potion hotkey, but curiously it did nothing.... I died miserably because I forgot to update the potion type on my hotkey, I had potions but not of the keyed kind.
A hard lesson, but I assure you it will be a remenbered one.
