    Cause of demise: Lost my second barb. Got caught by a frozen boss in a small hallway. Was frozen with bombs everywhere, my pot was ready and I was button mashing. Right beside me was a health orb. I died.

    Cause of demise: Working over the Butcher with HpSauce things got a little laggy and I couldn't nail the pot as quick as I needed. I was incinerated while running from fire and Sauce died shortly thereafter. We nearly offed the butcher, though.

    Cause of demise: Computer restarted without a clear reason in the middle of a fight. Not good when you stay logged on for about 10 seconds. When I came back, I was dead - Hall of Fallen Heroes mentions I was slain by a skeleton. I so want offline Single Player.

    Cause of demise: Defeated the Butcher earlier in the night and was doing the run again for practice/xp with Hobbsc. The fight went fairly well until he was below a quarter life, then Hobbs got stomped (see post 2 above). The Butcher was down to a sliver, I ran over to finish him off and boom, dead. Not even sure what happened! Apparently he hits hard.

    Cause of demise: Arrogance. Things were going so smoothly, I killed SK without a flinch. Had a boss tree with three minion trees. Health kept dropping from the poison but I was recovering with Revenge & the regen from Shout, but I should have run first time my health got to 25%. Killed the boss and almost had the minions dead but it was just too much poison. /facepalm.

    Cause of demise: not having any escape abilities and rushing into an enclosed area versus a Molten/Waller/Illusionist combo. I saw my health drop but couldn't get away, and when I paused at 10% hp with Near Death Experience passive (heals for 35% hp at fatal damage) on cooldown I knew death was inevitable.

    Cause of demise: Sleepiness, and retardness. I met magewraith rare pack with Nightmarish/horde affix. I was feared into wall but somehow i couldnt gather enough spirit to spread them out with a kick so i got stuck into some ****ty corner. I popped Serenity ... but still could not get out, either it was debris or not i could not gather why. I had all pack squeezed in 10 pixels where i stood and for love of god could not move out of this...

    I was staring for 5 minutes at the screen and instaed of being angry i was startled, monsters can block you? Or was it some piece of debris blocking the path. I will never know, and if i had better speed on weapons i would be able to stay alive with constant kicking and stunning the pack. Actually 45 lvl dagger was waiting in stash for me to reach it... And i didnt have problems with some ridiculous wasp combos earlier... if i had run into stairs not wall...

    Cause of demise: Stupidity. A molten arcane pack was sitting at the stairs in one of the tombs because I'd kited it back there, I decided to try to kill it because I figured I'd be able to get out if things went bad, but of course when you kite molten arcane away from the stairs, they leave a trail of molten lasers heading back to the stairs, so you really can't run backwards past them if you run out of space. At that point I was stuck trying to kite them forward through mobs hoping I could loop around, I got stuck, and I died... several lessons learned.

    Edit: While I still think I made some bad choices and that the death would have been avoided if I hadn't made them, I'm 99% sure that if Blizzard hadn't cut my Smoke Screen's effectiveness in half without telling anyone I would not have died.

    Cause of Demise: Lag. Game froze, 5-6 seconds later it jumps forward and I'm dead. Frustrating to die due to crappy servers, but better doing it now than in Hell or something Lesson learnt, no HC until servers are improved
