Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: MalyanTapir
Character level: 37
Character class: Barb
Character build: Frenzy, sword & board
Game difficulty: nightmare
Act: 1, Highlands
Death type: Lag Monster/PvM
Cause of demise: Hacked one of those poison dropper trees down to within a sliver of life at which time my swings seemed to be doing 0 damage. WTF, why won't this thing die? Oh crap, it's a major lag spike. ~15-20 seconds later deeds, but the Templar killed the tree. Was probably a poison bulb on the ground that killed me, char screen says slain by Unknown. Rather un-epic first HC death.

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: HellBeDamned
Character level: 56
Character class: Barbarian
Character build: 1H/Shield - Cleave/Hammer/Revenge
Game difficulty: Hell
Act: Act II Dahlgur Oasis
Death type: PvM
Cause of demise: Lost power in the house for just an instant. Wasn't in a really bad spot, about a half dozen skeletons that weren't doing much damage, but apparently enough. Logged back in to find I'd been killed by a Cursed Pilgrim.
Eschew obfuscation!

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: Haxed
Character level: 54
Character class: Monk
Character build: Deadly Reach, Conviction
Game difficulty: Hell
Act: 1, Leoric's Courtyard
Death type: Elite Pack
Cause of demise: Elite pack with arcane, frozen, and invulnerable minions trapped me on the bridge. Invulnerable minions surrounded me and I couldn't get out. It took me a long time to die but I eventually died. Had about 40k life and 300 resist all... I really miss instant save and exit from D2 HC.
If it took you a long time to die, you could have just popped your 4 sec immunity and log out. You'd only need to survive 6 sec of beating.
And yeah, this is why you need tempest rush
And here's mine:

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: JangBi
Character level: 60
Character class: Monk
Character build: Cookie cutter, deadly reach/blablabla
Game difficulty: Inferno
Act: 1, Halls of agony lvl 2?
Death type: Elite Pack
Cause of demise: Human error, encountered Hulks with Arcane, teleporter, something something. having doubts, chose to fight and kite instead of going into open room(i had cleared alraedy..) i took those very narrow passages (where only like 1 max 2 mobs can fit) thinking it will be a cake! I will kite them just to the entrance. Forgot most simple thing in the world : Teleporter. Always disregarded by me, this time punched me in a face, basically 1 hulk teleported in front of me trapping me into small space with arcane sentries. Suprisingly I survived for a long time almost killing the blocking mob (and if i would do this i would probably win they all were quite low by the end).
Hmm Alexander Berlin is it true? I always thought logging out panel dissapears when hit by the mob. And i had NDE...

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: Ygritte
Character level: 11
Character class: Female Barbarian
Character build: Dual Wield
Game difficulty: Normal
Act: Festering Woods
Death type: PvM
Cause of demise: First Hardcore character, died to a Returned Archer due to several successive power surges literally the instant before I engaged a rather large mob with an elite+minions mixed in. Knew I was dead the moment the power flickered. Worst part was that the day was bright and sunny with clear skies, no rain or strong winds to make me think twice about playing my HC character.

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: Amazonia
Character level: 60
Character class: Female Barbarian
Character build: Spear/Shield
Game difficulty: Hell
Act: 4
Death type: PvBug?
Cause of demise:
Well, that was a frickin' letdown...
Made it to lvl 60, hurrah. Did not want to get ahead of myself, so decided to stick it out in A4 hell and fiddle a bit with Neph Valor, etc.
Started the Prime Evil quest and went on ahead to go and kill stuff. Offed the Leiah spawns and Maghda spawns. Then encountered the first champ pack, a molten, illusionist, something pack of succubi thingies. Killed them and while reading the Neph Valor tooltip and collecting loot, noticed I got low on health. Turns out I see myself taking fire damage. Screen turns red and I'm like "waddafuq?". Poppped a potion and still life kept draining pretty fast, not an enemy in sight, coz all dead and the wp was far back...
So ended up dead... and annoyed as hell about it, tbh... Those last levels were a CHORE and the gear grindfest ahead of me to get ready for Inferno was not appealing at all either, so I guess I'm done for now.

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: Rosemary
Character level: 56
Character class: Demon Hunter
Character build: Devouring Arrow/Ball Lightning -Bow and Quiver
Game difficulty: Hell
Act: 2
Death type: PvM
Cause of demise: 37000 HP 5700 DPS had just finished a nice long co op run with friends. Went back a few zones to farm some gold and ran into a purple Fallen pack. Not a problem, I just started to kite like I usually do. Shoot, SS, shoot, vault, shoot...then vault and the vault animation did not stop. I knew I hit lag so without hesitation hit Esc then exit but the menu did not respond. I reached down and hit reset on my PC. Went to the kitchen to get some water for the inevitable. 2nd 55+ DH down starting my new one right now.
I refuse to "make a Barb" like everyone says because they are hard to kill. That's Blizzards job to balance the classes so when they do, my Demon Hunter will be ready no matter how many times it takes.

Diablo III RIP: Our Fallen Friends
Death Certificate
Character name: HpsMonkIII
Character level: 55
Character class: Monk
Character build: 1h/shield - Thunderclap
Game difficulty: Hell
Act: Act 1 - Tomb under the Cemetery
Death type: PvM
Cause of demise: Playing with my WD friend I was up ahead, spotted a pack of Imps with Fast, Arcane, Frozen. I turned to run, my friend (who had not seen the mob yet) had already started up the stairs, he spotted the rare pack and also started to run... but his Golem charged ahead of him and engaged. At that point we could have run out of the tomb and left them, but I thought "no problem, we have handled Arcane Frozen guys before & these are just Imps!"... So I charged back up the stairs to engage.
First freeze wave I was not able to dodge, so I popped Serenity and then was surprised when the second freeze wave came up almost immediately. I used Breath of Heaven to heal up after the 2nd freeze wore off, but again the 3rd freeze was already on top of me. Used a potion after the 3rd freeze wore off, but there was no hope at that point - in fact I'm not even sure if she was able to drink the potion before the 4th freeze.
I have since learned that the "Fast" mod which I never thought was that dangerous & regularly glance past, actually also affects the speed at which the freezing orbs are cast! Good lesson learned, but expensive price this time.
