Insert Image from file - no OK button

Trying to upload an image from URL works because there is an OK button at the bottom of the popup (cancel button too). However, trying to upload an image from file doesn't work because there is no OK button (no cancel button either).

I spoke too soon. Uploading an image from URL doesn't work either:

The CSS is messed up on the image popup you use to insert images. In the top right corner of it is a close button too but you can't see it. You have to mouse over it.

The button to upload the images from url is actually the text under the input box that says 'Upload Files' - doesn't look like the button but it is.

We should fix this.
