I was looking for this chance-to-hit formula everywhere and found it.
It look like this:
200*AR/(AR+DR)*Alvl/(Alvl+Dlvl)
AR - attack rating
DR - defense rating
Alvl - attacker level
Dlvl - defender level
minimum chance to hit is 5% and maximum is 95%.
But something is wrong. Game (LoD 1.13) and formula shows different chance to hit! Formula is outdated or it's a bug?

Normal Baal's defense is 313, not 208 (that's normal Diablo's defense), then it leads to a to-hit chance of 58%.. see here: http://classic.battle.net/diablo2exp...ct5-baal.shtml
