What is the diff between normal and forever skills? Do I have to max those skills or just put one point in them?
My forever skills do no change? Unless I reroll one? I would like to reroll for golum.
Locking skills, what is my benefit? If I lock nova, can I choose to keep dagger and explosion? Or will those get rolled in NM?
Also, what skills may I actually use in combat? just those rolled?

Forever Skills: 3 of them. You keep those in Normal, NM and Hell. The only chance to change them is one free reroll before you start Normal. Then never again (you can't save the reroll).
Normal Skills: 5 of them. They are rolled for Normal difficulty and after Normal difficulty, they are discarded and 5 new skills are rolled. The same is true once you finish NM: All 5 NM skills are discarded and 5 new ones rolled. The only way to keep one of the "Normal SKills" or "NM Skills" is to lock them. (see below)
You can put as many points in your forever skills and Normal/NM/Hell skills as you like. You may not pump synergies or passive skills that aren't Forever or Normal/NM/Hell skills though.

Locking a skill makes it available throughout all difficulties, in your case, locking Poison Nova would at least grant you a partially synergized Poison Nova throughout the whole game (because Poison Dagger is another Forever skill). You cannot lock Poison Explosion though - that skill would get rerolled with Revive, Iron Golem and Amplify Damage if you choose to lock Poison Nova. So you need some luck to get Poison Explosion again in NM and/or Hell.
You may use your Forever skills in Normal, NM and Hell.
Your Normal skills may only be used in Normal.
Your NM skills may only be used in NM.
Your Hell Skills may only be used in Hell.

Also for items is it the normal grade only or can a sash roll become a sash or demonhide sash for NM etc?
Yes, all items are just normal, you can use exceptional and elite versions of any item you get(exactly like your example)
@Cujo: You may not place point in a non skill other than a pre-req(1 point) So passives can be obtained by staffmods, as they aren't activated to use. For example my Iron Golem is a forever skill, I'm hoping to buy a wand with +golem mastery or summon resist passives so that I can at least pre-buff him before casting.

Well, an amazon's starting weapon just so happens to be a javelin! (Yes, if you accidentally throw the last one you can buy another stack)
But that was one of the previous years questions, and with the new way, you take that chance in if you did the whole non-bow thing as well.
In that case I will roll a Barb 2 x sword 1x axe
