Actually at this point it seems pretty obvious that the meteor is Sergio Ramos' penalty ball.
The only thing I don't understand about all this...who is Azmodan and where did he come from "I haven't read the books but I don't remember him from Diablo 2. He wasn't a Prime Evil was he? Was he mentioned in the book you find in the Black Marsh? (either that or Dark Wood). I thought there were only 3 Prime Evils (Diablo, Mephisto and Baal), but if there were actually 4, then that would explain it.

Azmodan is a lesser evil, like Andariel and Duriel. He was first mentioned in one of the lore books in D1, The Dark Exile.
In total there were 7 evils, 3 prime (Baal, Diablo, Mephisto) and 4 lesser (Andariel, Azmodan, Belial, Duriel) that ruled over hell. The primes were overthrown by the lessers and then Azmodan and Belial began fighting for control over hell.

I think the "Wrath" movie that just released as the 100% unlock significantly backs up the theory that it's in fact Tyrael falling.
So the end of days prophecy:
Tyrael – Archangel of Justice…And, at the End of Days, Wisdom shall be lost
as Justice falls upon the world of men.
Valor shall turn to Wrath-
and all Hope will be swallowed by Despair.
Death, at last, shall spread its wings over all-
as Fate lies shattered forever.
Auriel – Archangel of Hope
Malthael – Archangel of Wisdom
Imperius – Archangel of Valor
Itherael – Archangel of Fate
So the Angel parallel has already been stated before, but the "Wrath" movie reinforces that the falling angel is Tyrael, the angel of Justice.
We clearly see Valor has turned to Wrath from the movie, even the movie is named Wrath. Even if it was an assumption before, the Wrath movie only reinforces the theory that we can take the End of Days prophecy and directly translate them into the Respective Angels doing exactly what the prophecy says quite literally.as Justice falls upon the world of men.

So my personal spin on this theory is the following: The unity of the Angiris Council is now broken, and it's possible that Imperius himself, if not some other Angel or the rest of the Council, in his/their disagreement with Tyrael, may have thrown Tyrael out of the high heavens in the first place, causing him to fall and become the star that fell on Tristram. Possibly since he sides himself with Humans now, they may have said "If you side yourself with them, you no longer side with us, and thus you are banished. Go be with them, not us."
