I tried to put up a list of all the changes the Worldstone has gone through, here is what came out of it.

Is created in the death of Anu.
Lies in the center of Pandemonium.
The Pandemonium Fortress is built around it.
Is seized by Inarius and veiled from the sight of both the Heavens and the Hells.
Inarius binds himself to it, allowing him to draw upon its power.
Sanctuary is created and the Worldstone is housed in Mount Arreat.
The Worldstone is altered to hide Sanctuary from the High Heavens and the Burning Hells.
Inarius alters the resonance of the Worldstone to dampen the power of each succeeding generation of nephalem.
Lilith alters the resonance of the Worldstone to encourage the powers of the nephalem.
Uldyssian alters the essence of the Worldstone to make the powers of the nephalem grow faster.
Inarius destroys Mount Arreat.
Uldyssian severs the link between Inarius and the Worldstone.
All the alterations to the Worldstone are removed by Uldyssian. [1]
Somehow the Worldstone is again attuned to suppress the power of the nephalem. (probably as a result of the second pact made between the High Heavens and the Burning Hells)
Baal corrupts the Worldstone.
Tyrael shatters the Worldstone.

[1] Not specified anywhere but this quote hints at it:
"But the trouble, in Uldyssian’s case, in the case of all his followers, was that it had come too soon. Humanity — and he, in particular — had been thrust too fast into their destiny. That had been Lilith’s doing, the demoness too impatient to let the centuries lead men in the same direction. Uldyssian had not been given the opportunity to mature properly into his gifts."
Given that even Inarius could not unmake what Uldyssian did to the Worldstone, it stands to reason the Uldyssian would be the only one able to unmake what he did.

In the Sin War Trilogy it takes three generations and the help of Lilith for the nephalem to be able to express their powers, and that is with the Worldstone attuned to help in that regard. I don't know if there have been new nephalem since the Worldstone destruction, but if there are there seem to be a contradiction here.

In the lore panel they said that the Worldstone was protection Sanctuary from a large force of angels of demons from invading but in the Sin War Trilogy, even with the power of Trag'Oul added to that, a large angels army was able to invade Sanctuary.

I don't know if any of these issues are addressed in the book of cain. Maybe someone who has it could help us make more sense out of all this.

I finished the Book of Cain on Christmas Day, and here is what's said about the Worldstone's end:

Baal had fused his soulstone (in the book it's called "the Shard of Destruction") with the Worldstone, and his evil influence had already begun to spread through it. So when Tyrael chucked his sword into it, the stone was "shattered in a horrific explosion" that wiped out Baal's body, his army, and Mount Arreat. Cain also believes that Tyrael's "physical form" was destroyed in the explosion. He goes on to say that now "toxic clouds of ash and choking arcane dust hang like a shroud over the land".

Interestingly, he makes no mention of how he himself escaped all this devastation. If Tyrael wasn't able to get away in time, how did old Cain manage it?

Depends on how much of the mountain was destroyed, Harrogath, in my understanding lies on/at the foot of arreat. Absolutely could be wrong though.

If we interpret Bloody Foothills literally, then Harrogath would have been next to, rather than on, the mountain itself. I'm of the opinion that the town probably wasn't destroyed or its people killed, but it would most likely have to have been abandoned (the whole "toxic clouds" thing). I'm sure we'll get a definitive answer in act III.

Metzen was rather tight lipped about the current status of the Worldstone after Tyrael seemingly shattered it. We'll probably get more detailed info on it in the D3 or at least some breadcrumbs for the expansion.
