Is the ctc blizzard level random or does it depend on the item level?
Do monster actually have to hit me for blizzard to proc or can it go off on blocked attacks and whiffs?
Do I suffer the casting delay when blizzard goes off?
Is the spell synergized and is cold mastery applied when its cast?

The level depends on the item's stats. It'll vary from one belt to the next, but it won't change from one time it gets cast to the next. If the belt you have says level10 Blizzard, that belt will always cast a level10 blizzard. Item level refers to something different that has nothing to do with the ctc level.
Yes, monsters need to hit you. Blocked & missed attacks don't trigger the 'when struck' mod.
I'm pretty sure the delay does not trigger (for example, Rift-zealots can get a few FrozenOrb's flying all at once, so I can't see why it'd be different for this).
Yes synergies are added. CM is calculated at the time the damage scores (not when it's cast), and that will work too.

About the first question, I didn't explain it well enough. I was wondering, when the item is spawned is the level of ctc blizzard random or does it depend on the item level, like in eg. Hellslayer. I read somewhere that they get a higher ctc the higher their item level is.

Item level has absolutely nothing to do with the quality of the pre-generated mods that spawn on an item. Nothing at all. Ever. The values are determined randomly without any regard for the items' level.
You could have a perfect Skin of the Vipermagi dropped by NM Mephisto and a completely crappy one dropped by Hell Baal. The one from Hell Baal will be ~40 levels higher in item level, but that does not matter.

This is what I read..
To EasyGijs, I looked at my Boneslayer Blade, and I think it is safe to say that Hrus is correct, but the range should be 12-20.
My Bonelayer Blade looks like this:
Ilvl 55 => HBlvl 13
Ilvl 75 => HBlvl 18
Ilvl 82 => HBlvl 20
Ilvl 86 => HBlvl 20
Ilvl 99 => HBlvl 20
This must mean that the maximum HBlvl is 20 and the minimum is 13 or lower.
The Boneslayer has a qlvl of 50 and thereby must have a Ilvl of 50 or higher, I think this looks (from my 5 finds) like it corresponds to HBlvl 12.
So EasyGijs you should change the range of the Boneslayer Blade's HBlvl to 12-20. I think it should still be a variable in the perfection calculation.
My Hellslayer's looks like this:
Ilvl 86 => FBlvl 19
Ilvl 86 => FBlvl 19
Ilvl 86 => FBlvl 19
Ilvl 87 => FBlvl 19
Ilvl 88 => FBlvl 20
Ilvl 88 => FBlvl 20
Ilvl 88 => FBlvl 20
Ilvl 99 => FBlvl 20
So from this it can again be concluded that FBlvl 20 is max, but the min is somewhat elusive. The Hellslayer has a qlvl of 71 (Decapitator has a qlvl of 73) so the Ilvl must 71 or 73 (I am not sure here) but if this corresponds to FBlvl 16 is hard to say. But maybe someone has a lower Ilvl Hellslayer?

Now that you linked it, I vaguely remember that discussion. Having a quick look through some of my own stashed items, it looks consistent: Higher item levels = higher ctc levels.
My mistake.
but I'm guessing that it's a bug. :p
BTW, my snowclash from Baal (ilvl 99) only has a ctc level19 Blizzard. So good luck finding one with level20.

Hm.. maybe Snowclash isn't affected by this after all. If Baal drops only a level 19 ctc blizz.

I checked ~6 each of the Boneslayer Blades, Hellslayers, and Snowclashes that I have (couldn't think of other unique/set items with varying ctc levels, or I would check them too). All had higher ctc levels as the item level went up.
I'm guessing the level 19 ctc on Snowclash is as high as it goes, and item-listing sites like the Arreat Summit are incorrect. Or, more likely, this is a bug that has the side effect of limiting the minimum/maximum ctc level (and making item-listing sites incorrect in the process).

Kickers do suffer from Firestorm's cast delay on the Phoenix shield...
Does Blizzard hate sins? :(

I also recall using Hellmouth on some random Barb and each time firestorm triggered I couldn't even use my attacks for a brief period. That is why I asked about Snowclash, as I don't have the belt (yet!) I can't test it myself.
Maybe it depends on the spell..but I don't see how firestorm would have cd and fo wouldn't :dontknow:

FO (rift, chaos, etc) does have a delay, but for zeal and dtalon the delay is usually gone before the attack sequence finishes, so it's not as noticeable as with something highly interruptable like blade fury. Can't say for sure about Snowclash, but I imagine it's the same.
I'm not sure about the elusive lvl 20 Blizzard, but they're sometimes posted in ISO's on trade forums, and I've seen a few people who claim to have one for trade, etc.

The casting delay is supposed to be 1 second. Looking at these two screenshots, as well as the writeup, to me that would strongly imply the delay is NOT present on Rift's ctc Frozen Orb:

Looking at these two screenshots, as well as the writeup, to me that would strongly imply the delay is NOT present on Rift's ctc Frozen Orb:
The delay IS present. I have tested this extensively on single-player, and just made a rift on USEast to confirm the results on battle.net. Allow me to clarify.
The ctc orbs are unaffected by their own delays, so it's entirely possible to crank out 3+ frozen orbs 3 or 4 frames apart during a single Zeal or DTalon cycle, however the Zeal/DTalon skill will be unusable again until 1 second after the last orb was proc'ed. This isn't very noticeable unless the last hit in the cycle procs, causing your character to stop zealing/kicking for a moment between cycles.
The screenshots provided above are misleading, in that they were taken towards the end of the orb's lifecycle. Here are some I took just now:

So yes, Snowclash is almost certainly subject to a similar delay.

The delay on Snowclash isn't very noticeable from my personal experience, and I do believe you can get 20 CTC on one.
