Hi there, I recently found this funny little axe, and I simply cannot figure out why it stays as low a rlvl as it does.

I've been looking at affix calculators to no help at all.
It is a cool looking weapon though :)

I'm too lazy to work it out for you, but it's just a combination of one prefix and one suffix:
Prefix:
6 Poison-Damage over 2 Seconds (Rlvl = 1)
12 Poison-Damage over 3 Seconds (Rlvl = 7)
80 Poison-Damage over 4 Seconds (Rlvl = 15)
150 Poison-Damage over 5 Seconds (Rlvl = 26)
Suffix:
1 Poison-Damage over 2 Seconds (Rlvl = 1)
7 Poison-Damage over 3 Seconds (Rlvl = 3)
21 Poison-Damage over 4 Seconds (Rlvl = 11)
50 Poison-Damage over 5 Seconds (Rlvl = 18)
100 Poison-Damage over 6 Seconds (Rlvl = 25)
As you can see, there is no Rlvl here greater than 30.

It still doesn't make sense to me though.
I guess I'm just lacking basic affix/suffix knowledge.

Speaking of lazy, I've never gotten around to understanding precisely how two (or more) poison damages combine together but I do know the damages don't just add together.
So taking the "biggest" prefix (150 Poison-Damage over 5 Seconds) and suffix (100 Poison-Damage over 6 Seconds) from OMG's list and putting them on the same weapon will not produce 250 poison damage... it produces more than that.
There's a guide (or 15) around here that explains how multiple poison damages are calculated, but of course I can't find it at the moment. :p
Sidenote: I think that maybe I've read that poison durations DO add together for certain things, which could create your 11 second duration... but I could be wrong.

If an item has more than one poison affix, the damage per second is added together and dealt over the sum of durations. In your case it's a combination of the highest prefix and the highest suffix.
Total duration: 11s
Total damage: (150/5+100/6)*11=513 (see below)
The game makes the damage calculation in frames (1/25 sec) and rounds down here and there, so the result is usually off a small bit.

That was very helpful. Math usually kicks me in the grown every time I try to understand it, but this is very well written.

That rare Throwing Axe has spawned with the Toxic prefix and Anthrax suffix, and the total poison damage and length is calculated as krischan stated (at least for a single item with more than one affix). However, just as length is calculated using a more precise unit of measurement (1 frame = 1/25 second), so is damage (1 bit = 1/256 hit point). Poison damage consists of a bit rate and a frame length:
AFFIX BIT RATE FRAMES

TOTAL 479 479 275
Thus 515 (479 * 275/256) Poison Damage Over 11 (275/25) Seconds is displayed. Displayed damage is rounded to the nearest whole number for items, although it should be rounded down: a monster with 515 hit points wouldn't be killed by that damage (although it would be reduced to 115 bits, a fraction of a hit point).
