Sorry maybe a search could find the answer, but it is such a broad question.
How exactly does pvp work? Say I have a char that is in hell. Say level 80. I can join any game in diablo 2 and so long as its not in town, run up to the people and kill them?
If so how do I attack some one, I tried in my friends normal game earlier, he couldnt attack me and I couldnt attack him. I know you dont loose items or any thing but am just interested :)

You need to hostile a player while in town, use your party screen to do this, after that you can attack other players and get attacked by them.
Duels fall into a few categories:
pk - someone hostiles and kills a player that is not looking for a duel
bm pvp - players duel each other with no restrictions on rules for the duel, only dropping an ear counts, do whatever you want, expect very cheap tactics to be used (there are also players that use cheats in bm dueling but I will not be discussing that here)
gm pvp - players duel each other in a formal duel using gm (good manner) rules set up by leagues
If dueling gm it is best to agree with your opponent on what set of rules you are going to use for the duels.

Once you hostile someone they are hostile back to you automatically. So you could get killed trying to pk someone stronger than you.

There's quite a few different dueling leagues, with different dueling rules.
There's Low Level Dueling (LLD, 1-30, most are at 29/30 tier but lots of 18's, 9's, etc), Mid Level Dueling (MLD (31-49 usually, mostly 49 but not too many MLD's), High Level Dueling (HLD or just dueling, usually no level restrictions).
As for rules, like I said they vary greatly, but some of the most common:
-No third party programs/mods
-Only one piece of sorb OR maxres, and no res over 85 (ie, you couldn't use both a tgods and a wisp, but one or the other is fine. Some leagues allow amas and necs to use hotspurs)
-No potions except mana
-No Slow Missile, and no slow/decrep/hfreeze vs IAS based characters (debatable, some allow and some don't)
-No Lifetap/Ironmaiden
-No leeching from monsters
-No revives
-No charges from items
-Only a total of 451 poison damage from charms
There are also sometimes rules limiting bugged items which would depend upon your realm - it's pretty common to limit bmanas on EastNL, for example, usually to 13 total.

I was thinking that because you cant use skills! Any way is there a level limit like my 90 guy cant attack a 70 guy? Or my char in hell cant go in a NM game and attack some NM guys?

I am at work so want to find out so I can try at home, also knowledge = power. If I know things befor I try them, normaly you are more successful (sp) at them :)
No limit, at all. Hostile a person, your tp and their tp will close, and if you can find them, you can hurt them. They can unhostile you (so they can't hurt you) in an attempt to get you to stop, but they can't force you to, outside of leaving the game.
As long as you guys are over level 9, there are no caps.

Sounds like a good PvP system and one simular to my favorite game of all time! Can they use their partys portals? Also if I hostile one person they whole party does not get hostiled does it?

If you hostile a person, all portals you made disappear when you click hostile. You can make portals from then on, and they will stay there, only the initial one can disappear.
If you are hostiled to anybody, you can't take your party's portals from town. However, if you are out of town, you can take their portals back to town. Also, you can use portals you already set up.
If somebody hostiles you, any portal of yours doesn't disappear, and you can still take your party's portals back to town, but you still can't go from town out somewhere from another person's portal.
