Under what circumstances can a weapon lose durability?

When attacking physically?
When attacking magically?
When being attacked physically?
When being attacked magically?
When blocking?
When dying?
All of the above when it's the switch-weapon that is not currently equipped?

Is an ethereal caster weapon inherently bad? Is it even an issue if you don't get hit at all (assuming a perfect world)?

As far as i know weapons only loose durability when you attack with them so a staff/sorc orb or the weapon of a hammeradin can be ethereal because it won't loose dura.Just be carefull to not atack with it by mistake.

what about armor and helm? when I am under attack, will shield or armor or helm lose durability first? Thanks!

It's random I believe (well, as random as a machine can be), the game rolls a damage check on all pieces of armour

Weapon will also lose durability when using the smite skill, so watch for that.
