Upgrading Set Items

I have a noob question. Is it possible to uprgrade set items the way you can unique ones? If so, what's the recipe?

I hate to say it, but there's still the non-trivial issue of finding one which is worth upgrading.

I have an eth Ginther's Rift, but I won't upgrade it because I'm using it for leveling melee builds and a Grief PB is better anyway.
