PvM or PvP? Grief phase blade is best in both cases though.

For PvM, I'd go with eth glitched elite exile pally shield with 45 res (just in that general vicinity), grief, fort (or nigma if you gonna run stuff, COH if you need resists, shaftstop has good DR), dracs (or something else, you might not want that extra life tap, so go with something like rends), gores, guillames (or crown of ages or something), a raven frost, maras or anything nice, and a 2nd ring of ur choice.
