I have a smiter with HoZ. Hovering my mouse over the shield itself reveals "Chance to Block: 75%," but when I hover my mouse over "Defense" in the character attributes window (click "c") it only shows "Chance to Block: 33%". When I do Holy Shield it brings this number up to 44% even though on the HoZ the chance to block remains at 75%. Is max block determined by what the shield says or by what the attribute box says?

By what the attribute box says. The number that hoz says is only the shields base chance to block. Difrent shields have difrent base blocking.

Your chance to block is counted with the following formula.

Total Blocking = [(Blocking * (Dexterity - 15)) / (Character Level * 2)]

Blocking = A total of the Blocking on all of your items. (E.g: Guardian Angel, Twitchthroe, Any Shield, Holy Shield)
[] = Round Down

Note: Blocking on shield shown in game is capped at 75% while it may actually be higher. For a true account of most shields blocking ability... Look here forUnique, Set, and Magical/Rare shields. (for magical/rare, add the Blocking % from the suffix to the base blocking)
