Can someone point me to a guide (or tell me) that tells how/what it involves? As well as what items are worth upping :)
I was wondering the same thing this morning. I did a Google search for "Upgrading Diablo" and found this thread: http://forums.diabloii.net/showthread.php?t=440050
...which answered my question. My question being the exact same as your question.
But, to answer your question anyway, you can upgrade anything that's not elite. For example, Jared's Stone, Swirling Crystal and Dimensional Shard are the Normal, Exceptional and Elite versions of a weapon. You can upgrade any Jared's Stone into a Swirling Crystal, and any Swirling Crystal into a Dimensional Shard. A unique Swirling Crystal is an Occulus, and a unique Dimensional Shard is Death's Fathom. You can upgrade an Occulus to get an Occulus Dimensional Shard, but it will not turn into a Death's Fathom.
You should upgrade items to get more damage or defense, but keep in mind it will also raise the strength requirements. For example, I could upgrade my Skulder's Ire Russet Armor into a Balrog Skin which would increase the defense, but the strength requirement would be too high for my Sorceress to wear, so I leave it as Russet Armor.
Basically, just up items if you want to have more defense, attack, or extra belt slots.

also be careful with armours as when they are upped their base defence is randomised, and when they have a low ed they can sometimes end up with a lower defence than it started off with.
