it keeps happening to me in duals

Yea, at 1 hp left you turn human again after a hit, the next hit kills you tho...

It happens, but when you die in PVM you are probably pounded on by so many foes that you don't notice it.

It definitely happens in PvM and can be a life saver with IM. The reason is simple. There is no death animation for a weredruid so you have to change back before you die.
