That only refers to desynching in PvP.
To understand Desynch, you have to understand that Diablo is really being run in two different places. First, there's "clientside", which is what you see on your screen. Second, there's "serverside", which is what is actually happening. Sometimes, the client and the server won't agree on what's going on- this is best illustrated with the Amazon Skill "Lightning Strike". If you fire off a Lightning Strike, sometimes you will notice that the lightning arcs off to the right, but if you look and listen, you will see that the monsters getting hit are all on the left (away from the visual lightning bolt), and that the monsters on the right are really untouched. This is a form of desynch- the client thinks the bolt goes right, but the server thinks it goes left. When the client and the server disagree, the Server is always right (the client just displays what happens, while the server is the thing actually saying what is happening).
On the realms, Desynch can manifest in a number of different ways. If you've ever been running along when you ran into a "black wall" beyond which nothing exists, and then eventually you've sort of teleported back to a place you past a while ago, that's desynch- according to the Server, you never left that place, while according to the Client you kept going. Eventually, the client and server check to make sure you're in the right place, and the Client just pops you back where the Server said you were all the time.
If you've ever been out questing in PvM and you've noticed that none of your attacks are landing, that's because you're desynched, too. Your client says you're standing right next to a monster, but according to the server you're actually pretty far away, so when you swing, the server says you miss because you aren't in range. This is really, really annoying (obviously). To see where the server thinks you are, try opening a TP or dropping a gold piece. The TP or the gold will appear where the Server thinks you are. When you're desynched, the best thing to do is to leave the game and join another.
In PvP, some characters are capable of creating their own Desynch, so that they will appear somewhere other than where they really are. This is, of course, a big advantage because you can aim at where you think they are, but they won't actually be there.
