Help with Uber Trist info

I am new to these forums so I apologize in advance if this question is answered elsewhere. I have been hunting keys and opening areas, so far I have 3 meph brains and 2 baal eyes. Here are my questions:

1) Is there an order as to what area opens? So far it's been Furnance of Pain/Forgotten sands every time, not sure where/how the 3rd one opens.

2) Is there a way to make it so that the area where I get diablo's horn opens?

3) Are all of these areas, including Uber Trist, opened in act 5 only?

4) Am I doing something wrong?
