I've encountered this annoying thing with my lightning ama. Sometimes LF and CS misses, but I still use mana and the bolts are released.
So what causes it?
a) Lag
b) D/E/A
c) low FHR
d) low block rate
e) my momma

Strange since I have like 30 ping all the time.
It only happens when alot of monsters attack me, e.g. in WSK and there's about 23 gazillion flayers shooting darts at me and I'm trying to fry them with my LF.

It's D/A/E/Block. If any of those things trigger during (or even just before, in my experience) your attack cycle, you whiff.

Ah I see. It's kinda annoying to get 3-4 failed attacks in a row.
Would it help with more ias? I only got 20 from my 3/20 java gloves.

What breakpoint should I aim for? I got 55% FHR from Spirit.
edit: just saw that next breakpoint is 86. I would need 31% more FHR. Is it worth it?

Bad advice since Javazons tend to go into D/A/E/block recovery far more often than they go into hit recovery. FHR has absolutely no effect on the speed of D/A/E/block recovery.
Getting more IAS is an option, but it's far more effective to simply use your Merc, Valk, and Decoy to prevent yourself from getting hit in general.
I still haven't skilled IS, SM, Decoy or Valk yet. Just one skill each into the other passives. Maxed 4 light skills, but I'm considering putting 1 into the remaining 4 passives next.
Keep placing the decoy in front of you, or next to a pack of monsters, but right in front of them so they don't have to turn to see it. Then shoot away yourself. They willl go after the decoy. But you will need to use more skill points to keep it alive for long in hell. Strategically placed decoy and using a valk and merc is good advice.
I'm gonna start act3 hell soon. I usually join games in the wrong acts and solo. I haven't been close dying yet with only my merc to help me. I've always used valk before, but wanted to try without this time. I've decided to invest in a 1 point valk anyway just to get some heat off my merc.
I've got good eq so it's not really that hard:

coh
titan
spirit
3/20 java gloves
thundergods
natalya boots
mara
rare ring with mana leech
raven
torch
2 skillers
+ other charms
My merc uses Rattlecage (lol) and vampire gaze. Stuff I've found along the way. And also he uses a low req infinity since it's the only infinity I got. :)

Is it possible to for-go putting any points into D/A/E or will you be killed in hell? I find it highly annoying in nightmare when i get surrounded by quill rats and cant kill a single one because my passives keep triggering. I'm thinking about starting over and putting the defensive ability points elsewhere.

I'd say with max block and good resists you'd probably do just fine without D/A/E. In fact, to make a successful Fendazon you almost have to forego D/A/E.

Dodge/Avoid/Evade are passives skills that when activated put you into an animation of 8 frames length.
For certain melee builds its better to have no skill points in these and instead have a high fast hit recovery.

Sandstorm treks(20 fhr) are a good choice here.

Would skipping D/A/E for a PvP build be viable aswell? Considering Max Block & Enough Life?

From my limited experience javazons dont fair to well in PvP. Bowazons using guided do pretty well once you get used to the attack patterns of most people - and for them D/A/E is pretty important. Also, I can tell you that with my level 80 strafer, D/A/E has never caused the "failed" attack problem that constantly happens with my javazon, no idea why.
