For runes El-Ort you just need three of the same rune i.e. 3 El's to an Eld, 3Elds to a Tir, etc.

For runes Thul-Hel you need a chipped gem of a specific type i.e 3 Thuls + chipped topaz to an Amn, 3 amns chipped amythest to a sol, etc. (don't have the link readily available from work...)

For Io-Lem you need a flawed gem

For Pul-Cham you only need two runes and a specific gem (goes up to flawless, never need a pgem).

There are also lots of recipes for "upping" weapons and armor and for socketing weapons and armor as well. Check out the horadric cube page on diabloii.net (again, link not accessible from work :sad2: )
