Your vitality seems a bit low.
Gear suggestions:
-replace Soul Drainers with Dracs
-replace Wilhelm's Pride with Verdungos
-replace Cat's Eye with Highlord's Wrath (if you were relying on the dex on the Cat's Eye to maintain max block, just ditch the BK ring and use a 2nd Ravenfrost since the Highlords will cover the skill loss of ditching the BK.)

well if i think pvm i think 2 handed,its more dmg and u wont die anyway so tombreaver/ribcracker/teh botd gpa are all nice choices here

gloves:draculs
rings: 1 raven/1bk
armor:chains of honor/fortitude/enigma/duress/lionheart
belt:verdungos
boots:goreriders
helm:jalals
amulet:highlords wrath
switch: cta+spirit shield

try to hit the 42 fhr breakpoint atleast (shael your jalals for example)
inventory: max/ar/life charms or shapers+life if you arent that rich take 9 shapers/20life scs/torch/anni
