I've wondered this for a while after reading equipment items for low level characters; how do you keep it after smashing the orb? It always disappears, even if I don't smash the orb. Can it be kept in SP only, non-ladder, ladder? Thanks

You don't keep it after smashing the orb. You mule it to a lvl 1 character before you finish Act 3.

Actually, I believe you can keep it after act 3. What you need to do is get killed by Meph, and then don't collect the corpse. Kill Meph then, and continue on to Act 4, then grab your corpse. The flail should still be there.
