Guillaume's Face?

I keep seeing this helmet mentioned in almost every zealot guide. I haven't been able to find it in the item set or uniques list. What exactly is this item? Thanks.

The main reason for it being so useful for melee paladins is that it provides a 35% chance of dealing a crushing blow. That's dealing a fixed percentage of a monster's life (in contrast to dealing a fixed amount of life points) which is extremely useful for killing bosses with lots of life, like act bosses and it's mandatory to have a decent amount of it when fighting the Übers.
