Simply put, I have every rune I need to make Fortitude except one... Lo. Now I know I could just trade for one but I prefer to find it like I did the rest. What place is it most likely to drop? Can the Countess in Hell difficulty drop it or is it strictly a hell forge or Baal drop? All advice is welcome.

imho your best bet is to do hellforge a bunch.

Hellforge can't drop Lo.
So... kill monsters. Lots of them. Do high level areas.
Hellforge + Massive Hell Countess and transmute till your fingers go blue!

Trade all the items u dont need into runes! Cube runes to ists. Trade for a Lo rune! A Lo costs around 5 ists on Europe ladder. Dont price on US realms/asia realm though!

Countess' chance of dropping any decent runes in Hell is pretty low - pretty much the same as for the other monsters. Better off with doing a full mule HF rush, and then cube the runes or trade them for the LO (even if you'd rather not trade).
5 ists (using Dec's price guide here) vs 16 Ists or so to cube...
I know what I'd pick.
1: Make a LF zon
2: Put all your skill and IAS equip on her
3: Make her a Matriarch/Guardian
4: Get Wirt's Leg....
You see where this is going? Clear the level over and over again, in the thousands, and do it as fast as possible without overlooking good (rune) drops.

8 guls will give you a lo, so that's in the range of hellforge.

8 GULs requires quite a bit of hellforging, though.
Statistically, 44 hellforges will give you 4 GULs, 4 ISTs (another 2 GULs) and 4 MALs (1 more GUL).
Since you are also getting your nightmare forges you'll get a total of 8 UMs, which (if I'm not mistaken) makes another GUL.
So with "statistically perfect luck" you'll need 44 forges to be able to cube a LO.
With "real world D2 luck", you'll probably need around 500 forges...
On the plus side, that LO will be non-duped. I'm not saying it won't poof, though --- nobody knows how reliable the poofing works.

i got up to a lo last ladder through:
a found ohm
a made vex
1 gul
2 ists.
it takes a little luck and a lot of work.
i can't see myself making a cham through hf, certainly. but up to lo is certainly possible.
On average, 5500 countess runs solo in hell will net you 12 Ist and 1 Pul rune, cubing Pul runes, and up. Lem runes need 3 to cube, and don't pop that often anyway, so that may be more work than its worth.
12 Ist is 3/4 Lo rune, so around 7000 runs to get runes enough to cube a Lo. I know I could do it in less than 2 weeks of intensive running, single player with a good map. For realm play, I would consider getting a trusted friend and do those 44 mules through to hell hellforge, or rather 88 if your friend wants half the runes.
All of this is pure "statistic luck", however. There is a 1:320196 chance the countess will drop a Lo directly, on a solo run, and 1:229394 chance in a 8 player run... however, increasing players settings will make her quest drop decrease...

Thanks all. So in the end, the Countess in Hell(difficulty) can drop a Lo. ( its just extremely rare) It took me 6 months to get 0ne character to Hell difficulty. I don't even want to think how long it would take to get 44 other characters there, just for a Lo. Looks like I'll just play until I can cube one or it drops. Thanks for all the responses.

The response certainly wasn't about your advice regarding trading for a LO - quite agree there, as the odds of finding one yourself are so slim he might still be grabbing Wirt's leg till the cows come home...
The fastest way of getting a Lo (for certain) playing single player IS if you have a good countess runner available. (I can do that run in less than 40 seconds, and that includes loading times)
However, a *fast* cow killer could pop other, higher runes in the process of getting that Lo, so even if I could get a Lo from 10 days doing countess runs for five hours, I'd still favour cows because doing cows that much might just get me a Jah or some other rune higher than Lo.
I'd only go with countess if that Lo was *absolutely* necessary to a killer build. And I can't think of any builds cutting cookies any better than my already existing necros and hammerdins. Not to mention I have a "meph sorc", a "pindle sorc" and a "countess sorc" which couldn't be used for any other targets without dying...

Since english aint my native language I know that I make misstakes when writing from time to time! Besides I wrote that answer fast and did not check for "misstakes" so that the my answer should please YOU! No one else complained about my answer and the way i wrote it! Hi Guys,
I remember seeing it before, but cant find it......
Do you have a list of the runes that are available for each 3 difficulty countess runs, and the runes that are available for the 3 hellforges?
(or perhaps point me in the right direction)

That's easy...
Normal: El - Amn
Nightmare: Sol - Um
Hell: Hel - Gul
Chance for every rune is the same (1/11).
I woder if blizz at some point in beta testing had the first 11 runes in norm, next 11 runes in NM (as it is now) and then the LAST 11 runes (Mal-Zod) in hell... That could explain some of the rather crazy rune words in the beta that I have seen (with 2x Zod and so on...)

if they weren't so anal about drop%'s and rarity, they could easily put all dupers out of business if they wanted to. seriously, 4 zods in a month on a realm (a rough estimate) is way too low.

The traditional way to put dupers out of business, however, is NOT by making the rare stuff drop like crazy --- it is usually achieved by fixing the bugs that allow the duping.
There is generally two wyys to dupe: some race conditions/logic flaw allowing a player to get a copy of the item, or by using the classic crash technique (drop item, pick it up with another char, save other char, crash game -> now both chars have the item).
Obviously, anything that lets players crash the game should be fixed.
As for the race/logic flaws --- I don't currently see a reason why any such issue should remain in the game. Maphack is mostly based on serious design flaws of the whole D2 system, so it can't be fixed --- but IF the game required the "feature" that allows duping then there would be FAR more dupes/hacked items. In addition, bnet was originally meant to be a "safe" system: maphack is a passive program, as it only shows information delivered by the server, but it doesn't let you change things (you can see ilvl, but you can't set it. You can see the monsters, but you can't move them around). There is a very good chance that bnet design doesn't allow clients to change things other than by playing --- again, the conclusion is that none of the duping-bugs are required for bnet and that they can be fixed.
