does that affect the damage of hammers? spells? or just melee damage?

It affects weapon damage (melee and missiles), including skills which rely on weapon damage. For example, the vengeance paladin skill will benefit from it, but lightning fury won't.
It affects only physical dmg (but skills with dmg conversion, such as Berserk and Magic Arrow, but not Vengeance). The physical dmg done by the javelin of Lightning Fury is enhanced by +% dmg vs demons/undead when striking the correct monster type.

Is this a special exception for Vengance, or does Vengance generally ignore +ed% mods?
The elemental dmg is calculated using the weapon dmg plus min/max. Except damage +x, as on Grief. There's always an exception to any rule.
The physical dmg done by Vengeance is enhanced by the usual skill ed% and dmg% vs demons/undead.

Hmm, it makes sense that way, although not applying +damage from weapons doesn't.
So vengeance was a bad example...
