Question here - how does a combination of CoS and ITD work? ITD negates all defenses, and CoS lowers it. Does that mean that theoretically, a monster that is struck by an ITD item while CoS is on means that they have a negative defense? Or would defense just be negated to 0?

There is a sanity check that effectively prevents defence from going below zero. Note that ITD doesn't reduce all defence to zero--at the very least, players, mercs, minions, act bosses, and some superuniques are unaffected by it. So CoS would still be handy for those targets.
nevermind the other uses of CoS anyhow. I still think it'd be cool to have negative defense....kind of like negative resistances.
I think of it this way: Cloak of Shadows reduces their defense by a percentage of base defense. (Always, for everyone.) If ITD is on the character, then a check is made to see whether the calculated defense is used, or a value of zero.
Although, like BeB said, there is also a sanity check that will never let you lower defense below zero by any other means.

Actually , there is 1 skill that can make your defense go below zero. It happened on my sorc once. It's called Inner Sight. :D

I've seen the same thing, but the displayed defence has nothing to do with the to-hit check in this case.
