How is life leech calculated? If something has 7% LL per hit, does that mean 7% of damage done gets converted and added to your life, or is it 7% of the monsters hp, or something else? Will the LL be higher if you cause more damage? Is LL the same as life steal?

Yea, it is exactly like that. What say you have 100 damage, and you have 7 LL, then you get 7 life back. Therefore, the higher the damage dealt, the more health you get back. Obviously, having leech is good, but its not really worth massing up on. Also, it is useless in PvP, because of the reductions. Also, I think this is true but don't quote me, the amount of leech is based on the actual damage dealt to the monster AFTER Damage Reduction and other such mods are calculated. i.e. If you attack a monster with 30% damage reduction, and you deal 100 damage, then the 1k damage is reduced to 700, and then the 7 LL is calculated from that, meaning that you get 49 health instead of 70 health.

Actually, "dealt" is an important word here: if you do 1000-2000 damage and hit an act1 normal monster, you are actually dealing something like 10 damage, so you get 7% of 10 :-) That's why barbs can't really whirlwind in normal: their manaleech is way too low there (unless they happen to use some items with +mana after kills, of course).

In addition, lifeleech and manaleech work with physical damage only. If you are dealing physical and elemental damage, only the physical component is used for leeching (good examples for this are Vengance, Lightning Fury and Berserk --- in fact, Berserk has NO physical damage at all... so it not only puts your defense down to 0, it also leaves you without life/manaleech while using it...). It should be obvious that it is impossible to leech from physical immune monsters :-)

And, last but not least, from personal experience I'm pretty sure that the leeching is considerably cut down in hell. It also seems to be monster dependent --- not just "no leech" monsters, but some monsters seem to add a pixel to my manabulb when I hit them, whereas others fill up half the thing.

Leech is at 1/2 in NM and 1/3 in Hell, I believe.

Kargor, go to the Monsters section of the Arreat Summit. They all have a stat there called "Drain effectiveness" which tells you the percentage you can leech from them. Mephisto is 0 which means you leech nothing from him. Baal is 50, so after the hell penalty of 25%, it's another 50%, so you leech at 12.5% effectiveness on Hell Baal.
