According to Arreat Summit, it's possible to find a:
(+Skill Tree) Grand Charm of Vita (36-50 life)

Now, why hasn't one spawned with +41 life or more? I've only seen, heard, etc. of a +Skill Tree +40 life max, or if theres any other prefix, the max is 40. Is there an explanation I skipped over?

Vita (41-45 life) has a suffix level of 91. For 46-50 life it's 110. It's technically possible, but not bloody likely.

Yea, so does that mean only like baal can drop it?

Judging from monster levels I think only Pindle and Nithalak can drop it if it needs a high level, since they are the highest level monsters in the game (I think.)

Vita 46-50 is not possible (not even if the monster had a Mlvl of 1000)

DC is the only lvl 110 monster in the game. If his drops are changed he will be able to drop those charms
